Details You Need To Fill In The EPF Form 5

If you are in the HR department and in charge of filing the EPF Form 5 for new recruits, here are the details that you need to enter in the Form 5.

A simple method would be to create a Google Sheet with all the headers like Employee Name, Father’s Name, Account Number, etc. and share it with your employee/employees so they can easily give you all the required details.

General components of form 5A are

  • Name of company/establishment
  • Employer code/establishment code
  • Account number of employee
  • Employee name
  • Father/ husband name
  • Date of joining service
  • Duration of past service /work
  • The form should signed by the employer

Instructions to fill form 5A

  • Name of company/establishment
  • Permanent account number (PAN)
  • Code obtain as per act
  • Existing address
  • Proof of address
  • Copy of bank passbook/statement at least one is mandatory
  • Copy of power connection in the name of the organization
  • Date of inception of the establishment
  • Business type
  • Registration number
  • Ownership type
  • Employer Info
  • Nank name/Account type

  • Cancelled cheque copy

What is EPF Form 5?

If your company is registered under the EPF Scheme, you need to fill up Form 5 once a month with all the details about the latest recruits.

This notifies the EPFO that the benefits of the EPF scheme extend to the mentioned employees. Apart from that, EPFO also extends other schemes like the Employees’ Pension Fund and Employees’ Deposit Linked Investment (EDLI).

Also, keep in mind that Form 5A is only for employees that do not have prior registration with EPFO.

Steps To Fill Form 5A

Step 1: Vist the EPF Employer Portal Page

Step 2: Sign in with your establishment’s Username and Password

Step 3: Once you log in, click on Establishment in the Blue Navigation menu.

Step 4: Select Form 5A from the drop-down menu

Step 5: Enter the required details, and don’t forget to attach the supporting documents where needed.

Note: All PDFs should be less than 5MB. You’ll also need to attach the scanned copy of the account you mark as primary. This cheque shouldn’t be digitally signed and should be a JPEG, PNG, or BMP image with size less than 250KB.
